    Are you aware of the changing coverage and the criteria for Medicare’s Preventive Services or for the Prescription Drug Part D Enrollment? Do you know the ins and outs of the transition when you or your loved one is discharged from an acute-care hospital to a Skilled Nursing/ Rehabilitation Facility or to Home Health?

    We will be updated about how Medicare regulations and private insurances impact patients’ moves between different healthcare services. We will gain an appreciation for how important it has become for families or caregivers to be part of the team facilitating this transition, and how vital it is that this team communicates well and has access to all of the patient’s relevant health records.

* Jane Meyer, DSHS, Long-Term Care Ombudsman for Clallam & Jefferson Counties

* Lauren Newcomer, RN, BSN, Harrison Medical Center’s Director of Quality and member of a “Continuing Care Transition Project” will report on improvements in patient education at discharge and communication between providers at time of transfer.

* Lisa Holt MSN, MS, RN, Exec.Director of JHC's Home Health & Hospice will cover Medicare benefits and criteria for health care in the home.
Both speakers will also report on various quality measures required by Medicare.
